Care and Rehabilitation Center - LEŚNA POLANA

  1. The resort is perfectly prepared for the disabled and permanently lying.
  2. Patients are surrounded by the care of a internist
  3. In the event of life-threatening situations, an ambulance service is called, where an ambulance doctor makes decisions about taking or not a patient to the hospital
  4. In case of necessity to apply hospital treatment, patients are directed to the nearby NZOZ „Medicam” close to the resort.
  5. The referral is issued by the center’s doctor.
  6. The price does not include rehabilitation, which is possible on request, with the doctor’s consent, for an additional fee and subject to availability of places for rehabilitation.
  7. We divide patients according to their health and fitness, for groups A, B,C  – by differentiating the price of stay due to the different range of basic care services provided
  8. Patients with dementia diseases, including those with Alzheimer’s disease belong to group A,B,C,D regardless of the type of independence.
  9. We accept patients with catheters, probes, PEG, and tracheostomy tube.
  10. Insert / change catheter / probe are additional services – prices are given below.
  11. The replacement of the tracheostomy tube is carried out at the center only by an anaesthesiologist and is subject to an additional fee – according to the price list
  12. Assumption, replacement, removal of PEG is carried out only in hospital conditions, and the patient is directed by the center’s doctor.
  13.  The minimum stay in the center is 14 days.
  14.  The prices do not include the cost of medicines, pampers and specialist personal hygiene products.

All prices included:  

  1. Accommodation in a double room with bathroom, rehabilitation beds, TV and WiFi:
  2. 5 meals a day or an individual diet
  3. 24-hour care
  4. Internist doctor supervision over the proper course of the stay
  5. Measurement of blood pressure and temperature
  6. Occupational therapy from Monday to Thursday from 10, oo – 14, oo

Patient A – PRICE: 6500 PLN / 30 days 

Patient B with a tracheostomy tube and PEG or/and stoma PRICE 8500 PLN/30 days

Patient C  coma or vegetative state  PRICE 10500 PLN/30 days

Single  rooms  available on request and if possible. Additional fee + 50% of the package price

If necessary, basic diagnostic research (blood and urine) is carried out at the request of the center’s doctor, and is carried out in the laboratory of a nearby hospital and paid according to the price list of diagnostic services at the Forest Glade at the end of the settlement period. At your request, we will organize more complex diagnostic tests. Medical transport is also additionally payable.

ATTENTION:

The rate per day is calculated from the day of the patient’s arrival, regardless of the time. The day of admission is an organizational day on which the Patient is under 24-hour care of the staff, receives accommodation, meals, and the plan of his stay and the scope of care activities are determined. The doctor of „Leśna Polana” gets acquainted with the Patient within 24 hours from the day of arrival. Leaving the center by the patient on the last day of the contract is not subject to any time limits.

Implementation of diets in the center, for example only vegetarian or other due to beliefs, preferences or habits, taking your own supplements, mixtures etc … is possible only in agreement and with the consent of the center doctor, and their dosage is done by nursing staff. Independent use of any drugs or supplements, not included in medical orders, is prohibited.
